The Ohio Family Health Survey has created restricted access research datasets for 2004, and 2008 which include geographic indicators below the county level (zip code and census tract). Researchers may apply to use the research dataset by submitting an application to the Ohio Colleges of Medicine Government Resource Center. Researchers must have the approval of an Institutional Review Board prior to submitting an application.
Applicants must complete a Data Use Agreement Application, and Data Protection Plan. Applicants must also submit a signed Data Use Agreement and Supplemental Agreement with Research Staff. Upon approval. the signed agreements will be countersigned by the Government Resource Center and sent back to the applicant. After approval, if the project adds additonal research staff, they must submit an amendment to the Supplemental Agreement with Research Staff.
Upon approval and signature, the Government Resource Center will provide instructions on how to receive the requested datasets.
