AGE Quality Reports

The Ohio Department of Aging (AGE) provides statewide and individual facility-level reports each quarter to provide a comprehensive, data‑driven assessment of Ohio’s nursing homes. The Statewide Report highlights statewide and regional performance trends across the Ohio Department of Health’s eight Hospital Preparedness Program regions and includes detailed results across four key domains: Quality of Care, Quality of Life, Elder Rights, and System Performance. Facility-level reports allow for a data-driven, actionable analysis tailored to each facility’s performance.

Resource List

This full resource list includes a comprehensive range of supports, including programs and certification opportunities, quality improvement (QI) and safety toolkits, process maps, change packages, research findings, action plans, fact sheets, Excel Academy materials, and online trainings. The list will be updated quarterly to ensure the information remains current.
