Comprehensive Maternal Care (CMC) Activity Monitoring
Ohio’s Comprehensive Maternal Care (CMC) program, led by the Ohio Department of Medicaid (ODM), partners with maternal care practices to strengthen patient-centered care for Medicaid members, with a focus on prevention, care coordination, and improved access. CMC is a community-based, statewide program aimed at improving the health and well-being of moms, infants, and families.
GRC will support the implementation of CMC by:
- Reviewing how well participating practices are meeting program requirements for care coordination, behavioral health integration, and population health management activities.
- Providing technical assistance and monitoring performance improvement plans to help practices strengthen their delivery of care.
- Developing tools, surveys, and reports that help identify strengths, gaps, and opportunities for improvement.
- Supporting ODM in using data to refine program requirements and improve outcomes for Medicaid members.
