Comprehensive Primary Care (CPC) & CPC for Kids
Ohio’s Comprehensive Primary Care (CPC) and CPC for Kids programs, led by the Ohio Department of Medicaid (ODM), implement an alternative payment and care delivery model through primary care practices statewide. The programs promote patient-centered, comprehensive care for Ohio Medicaid members by advancing prevention, care coordination, population health, and access to care. Together, they help practices improve quality, achieve better health outcomes, and lower costs.
GRC supports the implementation and continued improvement of CPC and CPC for Kids by:
- Reviewing how well participating practices meet program requirements and implement key activities related to care coordination, population health management, access, and patient-centered care.
- Providing technical assistance and monitoring performance improvement efforts to help practices strengthen care delivery and meet program expectations.
- Developing tools, surveys, and reports that identify strengths, gaps, and opportunities for practice and program improvement.
- Partnering with ODM to use practice feedback, performance data, and monitoring results to refine program requirements, strengthen implementation, and improve outcomes for Medicaid members.
