GRC is excited to announce a new Quality Improvement opportunity for Family Medicine and Internal Medicine-Pediatrics providers!

Are you a provider interested in asking moms, “Care to talk about…your health, too?”

Care2 project aims to improve interconception care for mothers in Ohio by screening for risk factors at well-child visits and providing needed interventions.  Specifically, this effort seeks to screen mothers for depression, tobacco and vaping use, family planning and infant safe sleep, and provide treatment in follow up or connect them to appropriate resources.  Care2 is sponsored by the Ohio Departments of Medicaid and Health.
