Please join us on May 3, 2023, noon EST, for a one-hour webinar to learn about the Achieving Cardiovascular Equity Hypertension Quality Improvement Project. The QI project tested and identified best practices for providing care to patients with diagnosed hypertension, with a specific emphasis on care for Black patients.

This learning opportunity will address treatment algorithms, measures, project results, and publicly available resources that can be implemented at the clinic level. Healthcare providers, including clinicians and nurses, pharmacists, dieticians, quality improvement staff, and other support team members are encouraged to attend.

Objectives:
•    Identify and implement hypertension treatment algorithms for all populations
•    Identify key differences and similarities for treatment algorithms specific to Black patients with hypertension
•    Identify undiagnosed hypertensive patients in their medical practice
•    Translate how Social Determinants of Health can impact patient care and outcomes

Our Keynote Speaker is Joshua Joseph, MD, Physician and Assistant Professor of Medicine, Division of Endocrinology, Diabetes and Metabolism at The Ohio State University.
