Ohio Systems of Care Project ECHO for Youth Involved with Multiple Systems (SoC ECHO) is a partnership of the Ohio Department of Medicaid (ODM), the Ohio Department of Mental Health and Addiction Services (OhioMHAS) and the Ohio Department of Developmental Disabilities (DODD). This project will support youth with complex needs that cannot be met by a single state department. This will be accomplished through the development and delivery of trainings to professionals involved in the care and coordination of youth with complex needs on topics related to developing knowledge and skills to manage complex needs of children and families using the Project ECHO (Extension for Community Healthcare Outcomes) Model,™ a case-based learning and mentorship model. ODM, OhioMHAS, and DODD are collaborating with GRC, Northeast Ohio Medical University (NEOMED), Case Western Reserve University Center for Innovative Practice (CWRU CIP), and clinical and non-clinical experts from across the state to deliver this project. For more information and to see the series schedule, visit the Ohio Systems of Care for Children and Families website.
SoC ECHO Toolkit
About the Toolkit
What it is: This toolkit was developed for the Ohio Systems of Care Project ECHO for Youth Involved with Multiple Systems learning community. This toolkit is intended to help the learning community to “connect the dots” on key themes in the management of complex cases that arise from the biweekly case discussions and recommendations as well as to share associated resources from the sessions.
This toolkit is not medical advice and does not contain specific recommendations for individual cases. It represents trends observed from the Youth Involved with Multiple Systems cases in the ECHO sessions to help teams put more “tools in their toolbox” for consideration when approaching complex cases.
How to use: This toolkit is organized by key topics in the management of complex cases and provides the major themes observed to date from the biweekly ECHO sessions for consideration when working with youth with complex needs and any related resources. This toolkit will be updated every few months to reflect new information learned from the biweekly ECHO sessions.
