A Refined Approach to Improvement
The GRC applies QI science methods by creating toolkits to disseminate clinical best practices to participating sites, testing interventions utilizing Plan-Do-Study-Act (PDSA) cycles, hosting collaborative peer-to-peer sessions via action period calls, providing focused team coaching, leveraging Electronic Health Record (EHR) data for rapid cycle data feedback, and adopting clinical and administrative best practices which are consolidated into a final change package.
Implementation Across Care Settings
- QI initiatives within the GRC’s project portfolio address subject matter such as maternal and child health, dyadic care models, behavioral health, adult chronic disease, and substance use disorder.
- The GRC leverages a widespread, multidisciplinary network of health care organizations, universities, and state agencies to achieve improvements in population health.
- The GRC engages more than 300 clinical providers, practices, and subject matter experts statewide across our project portfolio.
- The GRC utilizes Electronic Health Record (EHR) data to identify real-time opportunities for improvement in the health care setting, and also links EHR data to state administrative data sets and other survey data to evaluate longitudinal outcomes.
Our Work
Our project portfolio is comprised of initiatives that aim to implement best practices across the system, provider, and patient levels in diverse health care settings.
Quality Improvement Capabilities
-
Quality Improvement Design
The quality improvement team at the GRC utilizes as adapted version of the Institute for Health Improvement’s (IHI) Model for Improvement to design and implement a robust QI framework for its initiatives.
-
Toolkit Development
The development and distribution of toolkits allows for the implementation of best practices across our initiatives. The GRC creates these resources leveraging evidence-based care standards and clinical subject matter experts.
-
QI Coaching and Site Support
The GRC’s QI team includes experienced coaches who guide and support participating sites through their improvement processes, providing education, data review, and utilizing PDSAs to achieve project goals.
-
Interactive Dashboards
The GRC creates interactive dashboards on an R-shiny platform which allow users to view QI data at incremental time points and stratify by variables including but not limited to age, race/ethnicity, and insurance status.
