Learning Health System Implementation: Building a Hub-and-Spoke Model for Hypertension Management Through the QI Hub
In July 2022, the Ohio Department of Medicaid (ODM) launched a statewide Regional Quality Improvement (QI) Hub program using a hub-and-spoke model to embed Learning Health System (LHS) capabilities in primary care. The Ohio State University (OSU) Hub piloted the approach across ten affiliated sites, aiming to raise overall hypertension control by ≥ 10 percentage points and promote more consistent levels of hypertension control across patient populations by June 2027. HEALing Communities Study: Data measures for supporting a community-based intervention to reduce opioid overdose deaths
The HEALing Communities Study (HCS) tested a community-based intervention in 67 communities across Kentucky, Massachusetts, New York, and Ohio to reduce opioid overdose deaths. This paper introduces the HCS measures for monitoring the intervention uptake, reports crude rates for benchmarking, and highlights the importance of interpreting jurisdictional trends in the context of state policies.September 2025 -

Applying the Urgent Maternal Warning Signs Initiative in a Novel Setting
Symptoms of urgent maternal warning signs (UMWS) may occur during pregnancy or after delivery and may have lasting effects or indicate a life-threatening situation if left untreated. The state department of health sponsored a quality improvement project (QIP) to broaden the reach of UMWS education beyond traditional clinical settings, to public health settings where prenatal and postpartum women are seen. A Prediction Model for Severe Maternal Morbidity and Mortality After Delivery Hospitalization
A prediction model for SMM or mortality up to 1 year postpartum was created and internally validated with information available to health care professionals at the time of hospital discharge.
