Multimodal Maternal Infant Perinatal Outpatient Delivery System (MOMI PODS)

The Multimodal Maternal Infant Perinatal Outpatient Delivery System (MOMI PODS) at The Ohio State University Wexner Medical Center is an innovative dyadic primary care program designed to close the critical gap in postpartum support for mothers and infants, particularly those navigating socioeconomic challenges or recovering from complex pregnancies. Available across nine Central Ohio locations, MOMI PODS provides coordinated postpartum care through a single‑visit model in which mothers and infants are typically seen together by the same primary care provider, reducing barriers such as time, transportation, and fragmented services. 

Continuous, Holistic Support for Moms and Infants

The program integrates a dedicated nurse care coordinator, a community health worker, and strong partnerships with Wexner Medical Center colleagues, Nationwide Children’s Hospital, and community organizations to ensure continuous, holistic support. MOMI PODS also collaborates with managed Medicaid plans to streamline enrollment for eligible families. Enrolled patients demonstrate markedly higher engagement across a breadth of clinical services and measures, and the program has been scaled both locally and nationally.

The GRC's Role in MOMI Pods

Since 2021, the Government Resource Center has led the program’s quality improvement and project management efforts, helping MOMI PODS evolve as a data‑driven and sustainable model of care. The GRC has guided the development of a comprehensive change package, created meaningful measures to track progress, and built dynamic dashboards and QI data systems that allow teams to understand performance in real time. This infrastructure has strengthened the program’s ability to adapt, spread best practices, and respond to emerging needs. In addition to these analytic and improvement functions, the GRC also coordinates monthly project meetings, facilitates collaboration across clinical and community partners, and supports ongoing stakeholder engagement to ensure that MOMI PODS continues to grow in alignment with the needs of families and the broader maternal‑infant health ecosystem.
